&#60;p&#62;The problem is that the preload-image is not hidden after the real image loaded, but remains to be in the background. &#60;/p&#62;
&#60;p&#62;Guess what happens if you use an image with transparency as featured image? &#60;/p&#62;
&#60;p&#62;Exactly. The spinning wheel remains visible THROUGH the featured image. &#60;/p&#62;
&#60;p&#62;Maybe something to fix for future releases (really hide the preload div once the real image gets loaded), but in the meantime: just use images without transparency (e.g. jpeg, or png re-saved without transparency)...&#60;/p&#62;

&#60;p&#62; sometimes the loading icon on the featured images does not go away, but keeps happily spinning over the already loaded image...&#60;br /&#62;
Happens in the blog overview as well as in single post view. &#60;/p&#62;
