For the first one (main menu item) select the checkbox "Use as Mega Menu". The second one must be a submenu item of the first menu item (so it's part of the mega menu). The second item will create the mega menu column title - enter it in the field "Mega Menu Column Title". The third item must be a submenu item of the second one. Select: "Use the description to create a Text Block. Dont display this item as a link.
