I'm having issues with this plugin.

I got it to upload and activate fine, only when I manually uploaded and did not use the wordpress uploader. When using the WP upload, on activate it would give me an invalid header.

None the less it's up and active, however when I go to my feedback page I get the error, Uncaught TypeError: Object #<an Object> has no method 'avia_feedback_box'.

It is actually being used on an internal intranet.

It poses a problem not being able to see the bug 1st hand, I'll try to recreate it but I can't make any promises.

UPDATE: It was a conflicting jquery library being added. I have resolved the issue. Thank you.
