I changed my contact page and it's url to just CONTACT instead of CONTACT US and now the header icon doesn't work, but I can't find where to fix it.

I don't really think that's related, but based on your post that the header icon on contact page is broken, I just checked the other pages if the same thing happened. Maybe it's just a coincidence. :)

Probably the only page that you were getting a recent version of because of W3 Total Cache. The others were probably older versions without the bug but it would have shown up later.
