I would like to make the following changes to the comment forms for each post:
‘Name’ to ‘Name*’
‘E-mail Address’ to ‘E-mail* (Will not be published)’
The third form is blank and I would like “Website” to be displayed there

My original Comments.php shows the following code but that’s not whats displayed on my blog. Example: My blog says “E-mail Address” and not “E-Mail (will not be published)” as shown here and same for “website”

Yes – in this case the field is empty. Search for following code in comments.php:
if ($comment_author == '') $comment_author = __('Name');
if ($comment_author_email == '') $comment_author_email = __('E-Mail Adress');
and change it to:
if ($comment_author == '') $comment_author = __('Name*');
if ($comment_author_email == '') $comment_author_email = __('E-Mail Adress*');
if ($comment_author_url == "") $comment_author_url = __('Website');
