1) You can adjust the image dimensions in function.php (I can't give you the exact code line because I'm not sure which image size you'd like to change). Afterwards use this plugin: http://wordpress.org/extend/plugins/regenerate-thumbnails/ to regenerate the thumbnails.
2) You can change the field captions, add new fields, etc. in includes/contact-form.php
At the moment the contact form supports text fields and textareas only. If you'd like to add advanced fields you can use plugins like: Formidable or Contact Form 7
