Im looking for to add a text box/type box in my pages... where my customer can type in what they want. Kinda like on the contact or check out page.
For example on the check out page it has - Name: [type box] Address: [type box].. How can i add something like this to my pages?

I have not worked with this theme. I just downloaded it so don't know a thing about it. But I have used FAST SECURE CONTACT plug in to create on line registration forms etc. It is set up for a contact page but you have options to go in and create other types of forms that are emailed back to you.
