Theme conflict using Avia Layout Builder with Gravity Forms Product Addons plugi

I have contacted both gravity forms and Woo Themes support and walked through trouble shooting. It appears that I cannot use a saved page template in a product with a gravity form.

First I created a page layout design and saved as a template.
Then I created a gravity form
Then I created a product.

In the product admin, I chose the template, then I chose the form in the dropdown menu for gravity forms add-on product, but it does not appear.

Upon further troubleshooting, I used the 2016 theme and all of the elements appear, the template and the form, but the form is in the product default layout. Also in the title bar it will not display the word Shop. Basically, I cannot use the avia page builder to insert a template and combine it with a form.
