Can you provide admin credentials so we can login and look at how you have it set up right now? Make sure to mark your reply as Private with the checkbox for Set reply as private.
Make sure that the first url that you put into that field points to where you have all of the video formats with the same file name (different extensions).

I’m not sure what you are trying to say. If you’d like us to look at your setup and see what might be happening we would need to log in live. You can use the video shortcode: https://codex.wordpress.org/Video_Shortcode to embed a html5 video (i.e. like here: http://www.shenom.net/2013/portfolio/ecco-schuh-kollektion/ ).
The lightbox does not support html5 video and tbh I doubt that prettyphoto will support html5 videos in the future. If we find a good, free open source lightbox we’ll probably replace prettyphoto with another lightbox but that’s not the case at the moment. Afaik even the popular fancybox script doesn’t support html5 videos without hacks ( http://www.fancyapps.com/fancybox/ )…
